Most posture statistics you find online are recycled. A number gets published once, gets quoted in a blog post, gets quoted again from that blog post, and by the fifth hop nobody can tell you where it came from or whether it was ever true. We went back to the primary research for this one, checked every figure against the paper it supposedly came from, and threw out the ones that fell apart.

What’s left is a fairly clear picture. Desk work is genuinely hard on the human back. The scale of it is larger than most people assume, the working-from-home shift made one specific part of it worse, and the interventions that actually shift the numbers are smaller and duller than the internet would have you believe.

Below are the posture statistics that survived checking, and what each one actually means if you sit at a desk for a living.

The Global Picture Is Bigger Than Most People Realise

Low back pain is not a minor workplace annoyance. It is the leading cause of years lived with disability anywhere in the world, ahead of depression, ahead of hearing loss, ahead of every other condition measured.

The Global Burden of Disease study published in The Lancet Rheumatology put the 2020 figure at 619 million people, with an uncertainty range of 554 to 694 million. Their projection for 2050 is 843 million, a 36.4% increase driven mostly by population growth and ageing.

The occupational slice of that is the part worth sitting up for. The same analysis attributes roughly 22% of low back pain disability to occupational ergonomic factors. Combined with smoking (12.5%) and high BMI (11.5%), those three modifiable risks account for about 38.8% of the total burden. Ergonomics is the largest single lever in that group.

Neck pain follows the same shape at a smaller scale. The companion Lancet Rheumatology analysis of neck pain counted 203 million cases globally in 2020, projected to hit 269 million by 2050, a 32.5% rise. The authors specifically flag that in high-income countries the elevated prevalence “might be attributed to a greater use of technological devices such as computers, laptops, and mobile phones”, which is roughly as close as global epidemiology comes to naming your laptop as a suspect.

Desk Workers Sit More Than They Think

Self-reported sitting time is a guess. Accelerometer data is a number, and the two rarely match.

Parry and Straker measured 50 office workers in Perth over seven days of waking hours and published the results in BMC Public Health. Their workers were sedentary for 81.8% of work hours. Outside work, across evenings and weekends, that dropped to 68.9%.

How the sitting gets distributed matters more than the headline. During work hours, 40.8% of measured time came in unbroken sedentary bouts of more than 30 minutes. Away from work, only 25.8% did. Office workers in the study accumulated over 12 hours a week in single sedentary stretches lasting more than an hour.

That distinction matters more than the headline percentage. Sitting for six hours in twelve-minute pieces is a very different physical experience from sitting for six hours in three long blocks, and the research on when posture actually collapses points squarely at the long blocks. We looked at that timing in more detail in when people slouch most: posture erodes steadily the longer a sitting stretch runs, so the hour-long block is where most of the damage lands.

Work is also roughly half the problem by volume. The same study found work contributed 36.5 hours of sedentary time per week, about 48.5% of the weekly total. Whatever you do at your desk, you are doing it for half of your sitting life.

Working From Home Made One Number Much Worse

The SITFLEX study, published in the Scandinavian Journal of Work, Environment & Health in 2025, used accelerometers to compare desk-based workers across different environments, including working from home. It is recent enough that most posture articles have not caught up with it yet.

Sedentary time was modestly worse at home: 378 minutes against 333 to 351 minutes in the two office setups, so around 27 extra minutes of sitting. Unremarkable, on its own.

The step counts are not. Desk workers in the office took 2,763 and 2,906 steps during the working day depending on the office type. At home, they took 1,257, less than half.

The commute disappeared, the walk to a colleague’s desk disappeared, the trip to a different floor for a meeting disappeared. What remained was a much shorter loop between desk, kettle and back. Interestingly, home workers made more sit-to-stand transitions (20.9 against 16.6 and 19.1), so they were shifting position more often. They just were not going anywhere when they did.

Where Desk Workers Actually Hurt

The body-region breakdowns vary between studies because they ask different populations different questions. A large survey of 1,185 office workers published in Occupational Medicine reported 12-month symptom prevalence in this order:

The neck and the low back sit at the top of essentially every version of this table, which lines up with the global burden data and with what we hear from users. Those are the two regions where sustained desk posture applies the most consistent load, and they are the two we have written about most: neck pain from desk work and lower back pain from sitting at a desk.

Be a little careful with any single number from this category. Prevalence figures for office-worker musculoskeletal symptoms range from roughly 40% to over 80% across the literature depending on the definition used, the recall period, and whether “any discomfort in the last year” counts. Anyone quoting one precise figure as the rate is overselling their evidence.

The Statistic Worth Actually Worrying About

Most posture statistics measure how many people have pain right now. The more useful question is how many of them still have it a year later.

A one-year prospective cohort study of office workers published in the Journal of Occupational Health followed people who developed new neck or low back pain and tracked what happened next. Of those with new-onset neck pain, 17% went on to develop chronic neck pain. For new-onset low back pain, 27% became chronic. So somewhere between one in five and one in four episodes stops being an episode and turns into a condition.

The predictors are worth knowing. For chronic neck pain, they were high BMI, frequent neck extension during work, high initial pain intensity, and high psychological job demands. For chronic low back pain, a previous history of it and high initial pain intensity. Two of those are about how much it hurts at the start, which is a decent argument for not waiting to see whether a bad week resolves itself. Two Posture Statistics That Don’t Survive Checking

If you have read anything about posture in the last decade, you have met this number: tilting your head forward loads your neck with 60 pounds, the weight of a small child, an eight-year-old, a bowling ball, depending on which article you found.

It traces to a 2014 paper by Hansraj in Surgical Technology International, which modelled cervical spine loads at various flexion angles and estimated about 27 kg at 60 degrees against 4.5 to 5.4 kg in neutral. The modelling itself is legitimate. Things went sideways in the interpretation that got bolted onto it afterwards.

A 2023 commentary in Rheumatology makes the case bluntly. The authors argue that “text neck syndrome is not a valid diagnosis since there is no evidence that the habit of using smartphones in a neck flexed posture is associated with neck pain”, and point out that cadaver studies show cervical spines resisting loads up to 244.94 kg, roughly nine times the figure that gets quoted as alarming. Living necks, they note, adapt better still.

None of that means head position is irrelevant. It means the specific scare-number is doing more work than the evidence supports, and the mechanism is not “your spine is being crushed”. Tech neck is better understood as sustained low-level muscular effort than as structural damage.

The second one is subtler. A cluster of confident-sounding percentages circulates in posture content: that 86% of workers sit all day, that 65% of desk workers develop back pain. We tried to trace both to a primary source and could not. They appear in article after article, always without a citation, or citing another article that also has no citation. They might be roughly right. There is no way to check, which is reason enough to leave them out, and a serviceable rule for the rest of the genre: a posture statistic with no link to a study behind it is decoration.

The Numbers That Actually Move

Anyone who has quietly concluded that posture is a lost cause should read this trial.

A three-arm cluster-randomised controlled trial published in the Scandinavian Journal of Work, Environment & Health followed 193 high-risk office workers over six months. One group took designated active breaks, standing up rather than staying in the chair. A second group made designated postural shifts while seated. A third group got a placebo seat pad and carried on as normal.

Neck pain incidence fell from 44% to 17%. Low back pain fell from 33% to 7-9%. Those are large effects from unglamorous interventions, and both intervention arms landed in roughly the same place, which suggests the mechanism is simply not holding one position for very long.

For context on the wider backdrop, the World Health Organization reported in 2024 that 31% of adults globally, about 1.8 billion people, did not meet recommended physical activity levels in 2022, with inactivity projected to reach 35% by 2030. The trend is going the wrong way. The trial above suggests the fix at an individual desk is more achievable than that trend implies.

What the Numbers Mean for Your Own Desk

Four things follow reasonably directly from the research above.

Break up the long blocks first. The 81.8% sedentary figure is hard to move much, because your job is your job. The 40.8%-in-30-minute-plus-bouts figure is very movable, and the randomised trial suggests that is where the benefit lives.

Get the screen up. Frequent neck extension during work was one of the predictors of neck pain turning chronic. A laptop on a desk guarantees it. A stand plus an external keyboard removes the reason to crane.

If you work from home, solve the step count separately. No chair fixes 1,500 missing steps. A deliberate walk, a real lunch break away from the desk, or a phone call taken standing will do more than another cushion.

Do not wait out a bad episode. High initial pain intensity predicted chronicity for both neck and low back pain. A fortnight of genuinely sore is worth acting on rather than sitting through.
